1. Should there be a rubber washer between the reservoir and its base?

Yes. It's a flat rubber washer.

2. How do I get the dust cap off the master cylinder piston so I can install new rubber seals?

You need to carefully pry back the pinched-in sections with a small screwdriver. Yes, it will look crumby when you're done, but there's little choice... just do the best you can not to distort the entire cap. The repair kit may come with a plain rubber cap, so you may not need the metal one anyway - depends on your kit... some come with the rubber washer instead that fits inside the cap, so you may need to re-use the metal cap. I've never seen a kit come with a new metal cap, though.
